2019 Club Re-charter Forms -
Clubs are encouraged to complete the rechartering forms (use either Word file or PDF) and return them via email to Sandy Fillebrown (see instructions). Please note additional paperwork is needed if your club does not have a 501(c)(_) determination letter on file. Also, we are asking clubs to designate which of their members is the club's Safe Sport representative (there can be more than one). This will be a requirement in 2020. We encourage you to read all pages thoroughly.
